We make every effort to give the best service possible to everyone who attends our practice. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint.
If you would like to make a complaint regarding the surgery or the services we offer, please complete this form and we will make every effort to respond to your concerns as soon as possible.
Please note that we have to respect our duty of confidentiality to patients and a patient’s consent will be necessary if a complaint is not made by the patient in person. All complaints will be treated as confidential.
The Parliamentary and Health Service Ombudsman website contains detailed information on raising a complaint about any aspect of the NHS in England. There is also a leaflet explaining the practice complaints procedure available at the practice, please speak to our reception team for this.
